New Instruction-Questionnaire Folder
New instruction folders, together with application blanks for membership in The Mother Church, have been sent to the entire Field. The new folder makes it much easier for applicants and approvers to become familiar with requirements for membership. It will take the place of the familiar pamphlet of instructions in use for many years.
The new form includes a brief questionnaire to be answered by each applicant. When both questionnaire and application form have been filled out, they are to be sent to the approver and countersigner in turn for signature, and then forwarded to the Clerk of The Mother Church.
Specimen application forms are reproduced in the new folder. Approvers should carefully read the questionnaire and check the application before adding their signatures. If the applications and questionnaires are forwarded promptly upon completion, it will greatly aid the Clerk of The Mother Church.
